I am Tammy Dziak, founder of Affinity Senior Transitions, and for many years I have helped older adults and their families navigate these very decisions.

My career has been devoted to senior care. My background includes, nursing studies, healthcare administration, ownership of a retirement community, a healthcare management company, geriatric care and senior housing referrals. That experience has given me the opportunity to see senior care from many different sides-but what matters most is the person and family sitting in front of me.

I understand that families do not call me because everything may be going well. They typically call because something is changing or has changed.

Maybe Mom has fallen. Dad is more forgetful. A spouse is exhausted from caregiving. A hospital discharge is approaching. Or perhaps you simply have that feeling that something just is not right anymore.

That is when having someone who is experienced beside you can make all the difference.

I help families understand what is happening, determine what their loved one needs, explore appropriate care and housing option, coordinate services and advocate for the older adult throughout the entire process.

Ans because I know the local senior care community, I can families move beyond simply asking "What places are available?" to asking the much more important question: "What is the right choice for the person?"
